We are a mobile assist center that are "boots on the ground" going out into the community and assisting veterans and community members experiencing homelessness and the issues that accompany it. We provide basic needs as ID cards, social security cards, food and clothing. We build a trust and then perform assessments on folks wanting help navigating the Sac System of Care for inpatient, mental health, job skills/education, veterans benefits and disability applications. We follow folks through the system, making sure each step or the way to their lifepath change is a plan for success.
